Innovation and technology have radically reshaped virtually every aspect of modern life – and healthcare is no exception, opening up new possibilities for prevention, care and treatment.
The team at NHS Horizons - a specialist team within NHS England driving transformation and improvements in healthcare - wanted to create a visualisation of what successful digital transformation would mean for community healthcare. Their aim was a clear, shared vision that would drive future action.

Solution
Recognising the power of visuals to inspire and engage, they reached out to Scriberia. To help create this, we involved the key stakeholders - experts with ‘lived experience’ of community care (patients, carers and families) and experts with ‘learnt experience’ (those who work in community health services) - to share their insights and shape the narrative.
Our artists crafted a vision map that demonstrated the potential of an inclusive and interconnected healthcare community. Focusing on the human connection, it captures the diverse nature of community services, representing different ethnicities, genders, ages, and abilities.
Results
The infographic serves as a visual touchpoint for the Horizons team to communicate their vision and drive conversation about the future of digital healthcare.
